Medication Therapy Management pada Penyakit Kronis di Layanan Primer: Kajian Pustaka Kualitatif-Deskriptif tentang Proses dan Luaran Klinis

Artikel ini bertujuan untuk mengkaji secara kualitatif deskriptif proses dan luaran klinis Medication Therapy Management pada penyakit kronis di layanan primer berdasarkan hasil studi pustaka. Kajian ini menggunakan metode penelitian kualitatif dengan pendekatan deskriptif melalui studi pustaka terhadap literatur akademik yang relevan dan mutakhir. Data dikumpulkan melalui penelusuran dan analisis dokumen ilmiah yang membahas implementasi MTM, peran farmasis dalam tim interdisipliner, serta dampaknya terhadap pengelolaan penyakit kronis. Analisis data dilakukan secara induktif melalui tahapan identifikasi tema, reduksi data, kategorisasi konsep, dan sintesis temuan untuk memperoleh pemahaman yang komprehensif. Hasil kajian menunjukkan bahwa MTM di layanan primer secara konsisten melibatkan proses inti berupa telaah obat komprehensif, perencanaan terapi individual, edukasi pasien, tindak lanjut terstruktur, dan deprescribing, yang berkontribusi pada perbaikan kontrol klinis, peningkatan kepatuhan, pengurangan masalah terkait obat, serta potensi efisiensi sistem pelayanan kesehatan. Temuan ini menegaskan bahwa MTM berperan penting dalam menjawab tantangan polifarmasi dan multimorbiditas pada penyakit kronis melalui pendekatan kolaboratif dan berpusat pada pasien. Secara keseluruhan, kajian ini berkontribusi dalam memperkuat landasan konseptual dan praktis MTM sebagai strategi penting dalam pengembangan pelayanan kesehatan primer dan dapat menjadi rujukan bagi praktik klinis, kebijakan, serta penelitian lanjutan di bidang manajemen terapi obat.
